Required information is listed below:
1. Signed rental agreement (Copy)
2. Copy of police report
3. Damage estimates for injury to persons or damage to property
4. Copy of current auto insurance policy and/or copy of credit card agreement used for rental (if applicable)
5. Take photos of the scene at the time of claim/accident/incident.
